Restriction vs. Occlusion – Selecting ICD-10-PCS Root Operations

Accurate ICD-10-PCS coding for procedures performed on orifices or tubular body parts (e.g., blood vessels, intestines) requires the ability to determine when to assign the root operation Occlusion and when to assign the root operation Restriction.

Please watch this presentation to learn about the objectives of the root operations Occlusion and Restriction and some procedures that meet these objectives.

Take Aways

  • The objective of the root operation Restriction is partial closure of an orifice or lumen of a tubular body part.
  • The objective of the root operation Occlusion is complete closure of an orifice or lumen of a tubular body part.
  • The root operation for embolization procedures may be either Occlusion or Restriction depending on the level of closure of the body part at the conclusion of the procedure.
  • Treatment of aneurysms is reported with the root operation Restriction.

About the Series: Coding in the Real World with Cari

Coding in the Real World with Cari delivers focused, practical education built for how coding actually happens—not just how it’s written in guidelines. Each episode breaks down one coding topic at a time, using clear explanations and real-world scenarios to help coders apply concepts with confidence and accuracy.

Led by Cari Greenwood, RHIA, CCS, CPC, an AHIMA-approved ICD-10-CM/PCS trainer with more than twenty years of experience in health information management, the series draws on deep expertise across inpatient, outpatient, and professional fee coding and auditing. Cari’s background in education, client support, and curriculum development ensures each session is both technically sound and immediately applicable.

From complex diagnoses to nuanced documentation challenges, this series is designed to strengthen decision-making, reinforce compliance, and bridge the gap between guidance and real-world application.

Quiz Answers and Rationale

Question 1: Placement of embolization coils to completely block off flow in a vessel is reported with the root operation Occlusion, but if coils are placed with an aneurysm just to fill it in, this is reported with the root operation Restriction.

Question 2: Stent grafts create a physical barrier the excludes the aneurysm from the normal path of blood flow. They narrow but do not completely close off the vessel lumen.
