Impella Pump Procedures: ICD-10-PCS Coding

Accurate ICD-10-PCS coding for Impella pump procedures includes building accurate codes for insertion and removal. Additionally, coders must understand under what circumstances these codes are reported and which associated procedures are also reported such as cardiac assistance provided in conjunction with pump insertion. .

Please watch this presentation to learn about ICD-10-PCS code assignment for Impella pump procedures.

Take Aways

  • Impella pumps are used to provide ventricular support during protected PCI procedures or during periods of cardiogenic shock
  • Insertion of the Impella 5.5 with Smart Assist® requires placement through a surgical conduit. Placement of the conduit is separately reportable.
  • A code for cardiac assistance is always assigned with insertion of an Impella pump
  • A code for removal of an Impella pump is not assigned when the pump is only used to provide intraoperative ventricular support
  • Repositioning of an Impella pump meets the definition of the root operation Reposition.

About the Series: Coding in the Real World with Cari

Coding in the Real World with Cari delivers focused, practical education built for how coding actually happens—not just how it’s written in guidelines. Each episode breaks down one coding topic at a time, using clear explanations and real-world scenarios to help coders apply concepts with confidence and accuracy.

Led by Cari Greenwood, RHIA, CCS, CPC, an AHIMA-approved ICD-10-CM/PCS trainer with more than twenty years of experience in health information management, the series draws on deep expertise across inpatient, outpatient, and professional fee coding and auditing. Cari’s background in education, client support, and curriculum development ensures each session is both technically sound and immediately applicable.

From complex diagnoses to nuanced documentation challenges, this series is designed to strengthen decision-making, reinforce compliance, and bridge the gap between guidance and real-world application.

Quiz Answers and Rationale

Question 1: Insertion of an Impella pump for provision of ventricular support only for the duration of a protected PCI is intraoperative support which is identified by assignment of qualifier value J Intraoperative.

Question 2: The correct ICD-10-PCS code assignment is 02HA3RZ, 5A0221D, 02PA3RZ. A code for insertion of a surgical graft conduit is not assigned because a conduit is only required for insertion of an Impella 5.5 device. Qualifier value J Intraoperative is not appropriate and a code for removal is needed because the device was left in to provide continued support after the angioplasty was completed.
