FY 2025 ICD-10-PCS Code Updates

The Center for Medicare and Medicaid Services has released the FY 2025 updates to the ICD-10-PCS code classification. This year’s updates are much larger than last year’s, with over 300 new codes being added.

The sections of ICD-10-PCS with new codes are:

ICD-10-PCS Coding Guidelines

Surprisingly, there are no updates to the ICD-10-PCS Official Guidelines for Coding and Reporting for FY 2025.

ICD-10-PCS Alphabetic Index

The addenda to the ICD-10-PCS index has 33 pages this year, 11 more pages than last year. Most of the entries added to or deleted from the index are related to changes in the tables such as the addition of new root operation, body part, device, substance or technology values, or the movement of devices and substances from one section of ICD-10-PCS to another. Some of the deletions and additions are made to correct errors.

The greatest number of updates are made within the code tables themselves, with New Technology being the section with the most tables with updates. The number of tables with updates per section and the number of new tables being added to ICD-10-PCS is shown here.

Sample of Significant Updates

Below is a list of some of the significant updates to the ICD-10-PCS code tables for 2025.

• Lymphatic Bypass - a new table is being added to allow for reporting of bypass of lymphatic structures.
• Sustained Release, Intraluminal Device, Drug-eluting – added qualifier value “2, Sustained Release” to the table for Dilation of Lower Arteries.
• Lumbar Artery Perforator Flap – added qualifier value “B, Lumbar Artery Perforator Flap” to allow for reporting of post mastectomy autologous breast reconstruction.
• Fiber Optic 3D Guided Procedure – a new method value (F) was added to the Other Procedures table to report use of fiber optic 3D guidance in performing endovascular procedures in the peripheral, aortic and aortic branch vasculature.
• Everolimus-eluting Resorbable Scaffold(s) – a new table for Dilation in the Cardiovascular System is being added to the New Technology section to allow for reporting insertion of Everolimus-eluting Resorbable Scaffold(s).
• Intraluminal Bioprosthetic Valve Leaf Splitting Technology in Existing Valve – a new table for Division in the Cardiovascular System is being added to the New Technology section to allow for procedures using this technology when performing a valve in valve, transcatheter aortic valve replacement (ViV TAVR).
• Introduction of Substances and Devices - There are 10 new substances and 3 new devices being added to the Introduction (XW0) table for New Technology Group 10.

    • Drug coated balloon for localized delivery of agent for percutaneous coronary intervention (PCI) to treat in-stent restenosis in coronary arteries
    • Temporary spacer system that uses irrigation with anti-infectives to treat periprosthetic fracture
    • Resorbable bone void filler for local restoration of bone weakened by osteoporosis
